L'action prend place seulement quelques minutes après la fin de la saison 2.
Après la mort de Pietro, son fils Genny prend le control de Scampia et Secondigliano et en profite pour prendre en main les différentes affaires en cours. Les survivants des factions de Naples, épuisés par la guerre et la pression policière, font la paix.
Cependant, les jeux d'alliances ne mettent pas longtemps à se réactiver et Genny doit lutter pour maintenir son clan au pouvoir.
